A long medical leave letter is a type of a letter which is written by a person to apply for a medical leave for a long duration from work. I, Jack Summers, the IT consultant working for this company am writing this letter to you to apply for a long leave from work due to medical reasons. I hope that you will grant me this permission so that I can get my eye treatment done.

I have had shorsightedness for a long period now but till now, it hadn’t bothered me as much as it started about 2 months ago. I consulted the doctors and they recommended me to go for the Lasik eye treatment to get rid of the number completely. The operation will last only for 30 minutes but I cannot use computers for about 20 days after that. Thus I request you to grant me leave from 10th May to 30th May. I would resume duties immediately after that and would make sure that I complete all project before going for the leave.
