Subject: Appeal for rejection of insurance claim number 79779/2013

Respected Sir,

I hereby would like to bring to your notice that a claim with number 79779/2013 was filed by me on the 21st of this month for an accident that I met on the 15th of this month. I have been undergoing medical treatment ever since and have sent a claim for all my medical expenses, however I have received a letter on the 28th stating that the claim was rejected as the insurance premiums were not paid and the payments are pending.

I have always been prompt on my insurance payments and have got all the documents and receipts to substantiate the same. I would request you to update your records to reflect all the payments, for my claim to be considered. I am also enclosing the premium receipts and my medical records for your review and understand that this should suffice for my insurance to be updated and my claim to be processed correctly.
